Sending Medical Records To Our Practice
If you are a new patient coming to see Dr. Randell for a consultation or you are transferring your care to our Practice, please obtain all of your medical records (including, without limitation, ultrasound reports and images, MRI reports and images, pathology reports, surgical records, etc), from your other health care providers. For your convenience, you can download the medical records request form from our website by clicking here. Please complete the form and send the form to your other health care providers. This form is only used to send records to our Practice. Please do not use it to request records from our Practice.
Requesting Medical Records From Our Practice
If you need to receive a copy of your medical records from our Practice, please send us your request in writing. The request can be mailed, faxed or emailed. Do not call the office to request your records. The request must contain your full name, date of birth, last four numbers of your social security number, email address, mailing address, telephone number and signature. Once we receive your written request, it will be processed in the order that it is received. All requests are subject to a charge consistent Georgia Law. Once the Practice receives your payment, the records will be sent to you. If you want your records sent to another medical provider, please provide us with the name and fax number of the other provider (in addition to all of the other required information). We will gladly send your records to another provider as a courtesy and at no cost to you.
